Paroxysmal nocturnal dyspnea (PND) occurs when there is an accumulation of fluid in the lungs (pulmonary oedema) at night. The patient is waken from sleep fighting for breath, a dramatic and frightening experience. Sitting on the side of the bed or getting up may relieve the breathlessness. Sometimes the patient will get up and open a window to gasp for fresh air.

The pain of pericarditis is felt in the center of the chest and like that of pleurisy is aggravated by movement, posture, respiration and coughing, but may be relieved by sitting forward.
